The provider is responsible to do the following:
Verify insurance eligibility of the client prior to initiation of treatment, including exclusions in terms of diagnosis and/or treatment modality.
To request sessions beyond the initial authorization, complete the "Initial Provider Evaluation/Treatment Plan Report". Submit form via Mines secure forms page at www.minesandassociates.com or by fax (303-832-9701)
prior to the completion of the last authorized session. This form is to be completed only once per treatment episode. This is the 1 st extension request.
Beginning with the 2nd extension request, complete the "Treatment Update & Extension Request Form". Submit form via Mines secure forms page at www.minesandassociates.com or by fax (303-832-9701) prior to the completion of the last authorized session.
Submit all Insurance billing to:
